We are able to provide sick (fit) notes when you have been absent from work due to illness for more than 7 calendar days.
If you’re off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill. Instead, they can ask you to confirm that you’ve been ill by filling in a form yourself when you return to work. This is known as ‘self-certification’.
If you have self-certified for at least 7 days and would like to request a sick (fit) note, please:
- fill out a sick note request form;
- telephone the surgery on 01531 825925; or
- visit the surgery and speak with one of our Reception team, Monday to Friday from 8am to 6.30pm.
