There are a number of services available online to all patients registered at Ledbury Health Partnership. You can order a prescription or book an appointment at a time convenient to you rather than having to wait until the phone lines at the Surgery are open. There is a wealth of information available to patients online via the NHS Choices website which allows you to search by condition and to find links to expert websites designed to help you manage your condition. All of these online services have been designed to make managing your healthcare as accessible as possible, any time of the day, anywhere.
The first step towards being able to order prescriptions, book appointments and much more is to register for online services with Patient Access. Patient Access is a partner of the NHS and connects directly to your medical record through our clinical system provider, EMIS. To register you will need to visit the Surgery with a form of photographic identification, such as your driving licence or passport, and a proof of address, such as a utility bill. You will then be given details of how to set up your Patient Access account on your home computer or mobile device.
Due to the current restrictions caused by the pandemic we are able to register you for online services without seeing you in person. Please contact the reception team for further details.

If you receive regular repeat prescriptions from the Surgery you will be able to order these items as and when you need them through the Patient Access system. Upon logging in you will see a list of all your repeat medications from which you can select, you are also able to add a small message to the Surgery team about your prescription request. This request is then sent to the doctor to approve and sign, if for any reason the doctor is not happy to issue your repeat medication a member of the Surgery team will notify you.

Patient Access connects to our clinical system EMIS which means that you are able to view your health summary information, such as any allergies or immunisations that you have had. There is an additional function available to you at request for you to view the coded information held on your medical record. Coded information refers to the items in your record that have been assigned a code in the computer system, for example any diagnoses you have had or test results. Having access to this information easily at hand can help you to manage your medical conditions more effectively.
To gain access to your medical record, even if you already have a Patient Access log in, you will need to fill in a request form and provide the Surgery with the necessary personal identification for us to ensure we are providing this access to the correct patient. Prior to access being granted to you the doctor will need to review your record. You will then be provided with a log in for which you must choose a unique password that only you would know. This will ensure that it is only you that has access to the sensitive personal data held on your medical record.

It is important that we always have the most up to date contact details for you in case we need to get in touch. Through Patient Access you are able to update your address and telephone numbers if they change.
Contacting patients via text is becoming a more popular way for people to receive information from the Surgery. Having your up to date mobile number is a quick and easy way for us to get in touch with you if we need to.
It is common for children to have the mobile number of their parents noted on their records. If you get your own mobile telephone please remember to let us know so that we can update your contact details accordingly.

If you’re a patient at our practice you can now use the new NHS App, a simple and secure way to access a range of NHS services on your smartphone or tablet. You can use the NHS App to check your symptoms and get instant advice, book appointments, order repeat prescriptions or view your GP medical record, if you have requested access to your information. If you currently do not have access to your records, please ask a member of the Reception Team who will be able to provide you with all the information you require to register.
