Ledbury Health Partnership serves the community of Ledbury and the surrounding areas. If you are interested in becoming a patient at Ledbury Health Partnership the first thing that you need to do is check if your address is within our catchment area. If you are not within our catchment area further advice is available by visiting the NHS website.

New Patient Registration forms

The registration forms need to be completed in full and returned to the Surgery in person for registration to be completed.  You will need to bring with you a form of photographic ID (driving licence or passport) for proof of identity and a utility bill or bank statement for proof of address.

Every patient at Ledbury Health Partnership has an allocated named accountable general practitioner who is responsible for the patients overall care at the practice.  You are still free to see any GP that you wish to in the practice but your named accountable GP will take overall responsibility for the co-ordination of your care.  If you wish to know who your accountable GP is please ask at reception.  If you have a preference regarding your named accountable GP we will always try and accommodate this request although this may not always be possible if a particular GP already has an oversubscribed patient list.

NON-ENGLISH SPEAKERS

If you are a non-english speaker you are able to book an interpreter to support you during your appointment. At the time of booking your appointment please ask the receptionist to organise an interpreter for you. Please be aware that interpreters are booked in advance so if your problem is urgent we may not be able to provide an interpreter.
